
Integrated Health CareIn response to today’s evolving and often fragmented health care environment, Hospital Sisters Health System has partnered with a variety of providers in our care integration initiative. Our strategy is to unify the key components of health care delivery in our Illinois and Wisconsin communities—linking physicians, patients and technology.
- Patients—Care integration means seamless care for patients, from the time a person enters the system as a newborn through to the end of life. Care integration meets the needs of today’s health care consumer, who is more likely to receive care outside the traditional hospital setting. Care integration embraces this reality and brings continuity of care at every point of access in our System.
- Providers—Care integration means partnerships with physicians and other health care professionals. Known collectively as our “physician enterprise,” this community of caregivers forms the backbone of our integrated care network. In addition to HSHS Medical Group, HSHS has built a number of partnerships with large multi-specialty groups, mid-sized medical groups, small group practices and solo practitioners.
- Support Structure—Care integration means integrated technology, including electronic medical record (EMR) capabilities to improve quality and prevent waste. For professionals in HSHS Medical Group, ambulatory EMR will be interfaced to the hospital EMR through the use of Medicity. Integrated technology links the physician’s office, the ambulatory care center, the hospital and even the patient’s bedside—and allows the physician to focus on healing rather than paperwork.

Prevea Health, Wisconsin
HSHS has worked as an equal partner with Prevea Health for more than 15 years to ensure our patients have access to truly coordinated, integrated care in Eastern Wisconsin.

Prevea Health has taken significant information technology steps that deliver real value in support of HSHS care integration efforts.
Prairie Cardiovascular ConsultantsPrairie Cardiovascular Consultants is a group of cardiologists based in Central Illinois. Prairie physicians are nationally recognized as leaders in cardiovascular care and represent an important partnership for HSHS.
Prairie is a national leader in the innovation and development of new diagnostic and treatment procedures and continues to advance medicine with a very active research program.
